Hospital to home
When a discharge is driving the referral, call rather than fill in a form. We have started supports within 48 hours for hospital discharges and urgent placements. We cannot promise that every time — but we will prioritise the review, tell you honestly what is possible, and move quickly.
What to have ready for the call
- Location and timing — suburb, and the planned discharge date
- Support complexity — clinical, behavioural or environmental needs, and any behaviour support plan
- Roster shape — hours, overnights and ratios needed from day one
- Worker requirements — gender, language, skills or continuity needs
- Plan details — management type, funded lines and plan end date
Do not wait until you have all of it. Partial information is fine — we can fill the gaps on the phone.
What can slow it down
Waiting on plan details or funding confirmation, or finding the right worker match for specialised or overnight support. We will tell you where things are at rather than leaving you guessing.
If complex care is involved
Our Clinical Lead, a Registered Nurse, reviews the referral and tells you what training or sign-off is needed before the first shift.
What happens after you call
We prioritise the review
A discharge goes to the front of the queue. Every enquiry gets an answer within 1 business day.
We tell you what we can staff
Plainly, with a real start date rather than a vague ‘soon’. If we are not the right fit, we will say so and point you elsewhere.
Workers read the care plan first
Routine, preferences and known risks are read before arrival, so the person never has to explain themselves from scratch.
Same-day contact when things change
An incident, a health concern, a difficult shift — you hear from us the same day, and coordinators get a written report every fortnight.
Where we can help
If the person’s suburb is nearby rather than listed, still call.
